The HôME Pilot Program has been established to assist HACC eligible people in a defined geographical area to maintain and improve their home management skills and resources.
The program is designed to be an early intervention option for people who are having difficulty with home management tasks and are therefore potential Home Care clients.
The goal is to decrease the level of difficulty the target population and their carers are experiencing in managing their home management tasks.
What clients can expect
Eligible clients will be assessed by an occupational therapist, who will develop an individualised program in collaboration with the client and other key people (eg. family, carers if appropriate).
This individual program may incorporate recommendations for:
- task or environment adaptation
- accessing community services
- training in specific skills
- involvement of resource people (family, carers, home care workers, health care professionals).
The program will be implemented and monitored by the occupational therapist.
