Our Young Adult Kidney Care clinic offers care to young adults aged 16 to 25 years who have had a kidney transplant or have advanced kidney disease, through a dedicated inter-disciplinary team. This team delivers care specifically tailored to the needs of young people supporting emotional, social and educational needs. This clinic is different to regular adult outpatient clinic appointments as patients meet and other young people with similar exeriences of kidney disease or transplants, and can be involved in activities and social events together.
