Our Mater at Home social workers are able to provide clinical support and intervention to clients to address social, emotional, physical and financial disadvantage where these issues are impacting on the client's wellness and capacity for independent living.

Mater at Home offers short term assistance to clients experiencing a complex range of problems including:

  • counselling and support for carer stress
  • information and referral to community services
  • grief and loss counselling
  • assistance with access to housing and income support
  • support and counselling for anxiety, depression and adjustment to chronic health conditions
  • support with future planning: Advance Care Planning including identifying Next of kin  (NOK), completing Enduring Power of Attorney (EPOA) and Statement of Choices (SOC), QCAT applications
  • support with squalor syndrome and hoarding
  • advocacy and support with elder abuse
  • support with social isolation – patient at risk of self-neglect, linking with community social supports
  • adjustment to illness, support for patients and family
  • end of life support (bereavement /practical assistance)
  • mindfulness 
  • assistance with other issues impacting on the client’s health, wellness and ability to live independently in the community

Our paediatric social workers can assist with:

  • short term counselling for children, adolescents or families
  • support and advocacy with and for the child, adolescent and family
  • assistance with complex psychosocial situations in the home
