If you are a Mater staff member with a current Mater ID (Payroll Number) and you have received your flu vaccination at your local pharmacy or GP, please complete the form below.

Flu vaccination is the best way to prevent catching and spreading influenza.

Thank you for doing your part to keep Mater People and our community safe this flu season.
