The Mater at Home team provides home and community-based Allied Health and Nursing services to both adult and adolescent patients.
The service is flexible and provides a rapid response to residents that live within a 40 km radius from the South Brisbane campus.

Public patients
- The service is flexible and provides a rapid response to patients that live within the Mater at Home catchment area (40 km radius from the Mater South Brisbane campus).
- Service areas, eligibility and referral criteria vary according to the type of program referred to. A small co-payment may be required for some services. For further information regarding a specific referral, please call our office on 3163 1760.
- Referrals are accepted via phone, fax or email
Private Patients
- All Mater at Home allied health and nursing services are available on a fee for service basis for both adults and adolescents. The service is flexible and provides a rapid response to residents that live within a 40 km radius from the Mater South Brisbane campus.
- Eligibility, referral criteria and cost to the client vary according to the type of program referred to. For further information regarding a specific referral, please call our office on 3163 1760.
- Private patients will incur a fee for appointments

Patient referrals are triaged according to prioritisation guidelines and are booked for appointments accordingly. Referrals can be emailed or faxed. For questions or more information, please call 07 3163 1760 Monday to Friday, 7:30am to 4:30pm.

Mater Aged Placement Service (MAPS)
Support for transition to residential aged care.

Transition support for patients and their loved ones
Do your patients need support transitioning into a residential aged care facility? Finding the right placement for them or a member of their family can be an overwhelming experience.
Based in South East Queensland, Mater Aged Placement Service (MAPS) have expertise across the health and aged care systems and will work with families to find a suitable aged care home and support this transition. Our team is made up of experienced professionals who work within the Australian Association of Social Workers code of ethics, alongside caring support staff who will assist with more practical aspects of the process. We understand the admission process of aged care facilities, and we have extensive experience in liaising with older people and their families.
What we do
- Assessments of your patients' aged care requirements
- Identification of suitable aged care facilities
- Liaise with and refer to Aged Care Assessment Teams (ACAT), who are responsible for assessing eligibility for aged care placements
- Assist with the completion of paperwork, including application and asset forms
- Ongoing support until a suitable aged care placement is secured
Mater Aged Placement Service is a service within Mater at Home. We have no formal affiliation with any aged care facilities and are not paid or remunerated by such facilities.
Fees
MAPS is a private offering and fees will apply for use of this service. Not all placements are the same and patients with more complex needs may require additional services.
