For Health Professionals

To refer a patient please complete and submit the online form below. Alternatively, you can download and print the referral form, then fax it to 07 3010 5745 or email it to info.wellness@mater.org.au. A Mater Health & Wellness staff member will then contact your patient directly to arrange appointment details.

For assistance, you can also phone our clinic on 3163 6000 and follow the prompts. Our reception is available Monday to Friday, 8.00am–4.00pm.

Parking and transport

Mater Health and Wellness is a short walk from Mater Hill Bus Station and 10 - 15 minute walk from South Bank Train Station. Parking is available at a number of car parks at Mater. The closest carpark to Mater Health and Wellness is Hancock Street carpark.
