What does Discharge mean?

  • Discharge means that the specialist team are returning your care to your GP for ongoing management of the condition for which you were referred to the Specialist Outpatients clinic. 
  • Your specialist clinical team will make the decision once they are confident that your condition can be best managed, ongoing by your GP.

Does my GP know?

  • Your clinical specialist will report back to your GP with your results, diagnosis and/or treatment, and your ongoing management plan. 
  • It is important that you continue to see your GP for all your health care needs.

I still need to see someone

Your GP is best placed to manage your ongoing health care needs. You should make an appointment to discuss any concerns you may have with them. 

I was admitted and need an appointment 

  • Your specialty team will arrange for your post-admission follow up care.
  • This appointment will usually be in the Specialist Outpatient clinic.
  • You will receive a SMS or letter notifying you of your appointment details. It is important that you confirm your appointment as requested in your SMS or letter.
