What I need to bring to my appointment

Bring the following items with you to your appointment

  • Medicare card
  • Pension card / Healthcare card / DVA card
  • List of current medications (prescription, over the counter and herbal medicine)
  • Your appointment letter
  • Any relevant test results, reports and medical scans such as x-rays, scans or ultrasounds
  • Glasses, hearing and mobility aids
  • Your GP contact details
  • Refreshments
  • Any specific tests or materials requested in your appointment letter
  • Reading materials or quiet activities
  • A list of any questions that you wish to ask the specialist

Support person

You are welcome to bring a support person to the appointment.

In most situations, for patients under 18 years of age, the parent or guardian will need to attend the appointment.

If you are attending the ophthalmology (eye) clinic you may not be able to drive home.  Please bring a support person or plan alternative transport. 


I have an endocrinology annual health review

Please make sure you have completed pathology test and your optometry review.

If you have received a request for x-ray, CT, MRI or ultrasound please complete them with QXR before your appointment.
