Mater Aged Placement Services

When it comes time to consider aged care placement for a loved one, there are some things you can do to help make this process a smooth transition for all involved:

  • Plan ahead and complete an Enduring Power of Attorney for your loved one, provided they have a My Aged Care Support Plan with approval for Permanent Residential placement
  • Talk openly about care options with your loved one, as well as all other family and friends involved; discuss current care needs and time frames
  • Emphasise your loved one’s safety and wellbeing throughout the process

Mater Aged Placed Services works alongside clients and their families to simplify the transition into an appropriate facility and provide professional advice and understanding when settling in.   • Identification of which aged care facilities may be suitable, and suitable to the needs of the extended family

  • Liaison with and referral to aged care assessment teams, who are responsible for assessing eligibility for aged care placements

  • Work quickly and efficiently to provide timely solutions
